Screening and Assessment
Tools
The CHSD has developed a number of
screening tools that prompt assessment, referral and
broader needs identification. These tools have been
implemented in a series of projects, beginning with
functional screening and assessment in Home and Community
Care and Aged Care Programs.
Further developments have taken place
in Victoria, South
Australia, NSW and Queensland,
where the functional screen is part of ongoing needs
identification in primary Care. The suite of tools is
called the "INI" (Initial Needs Identification)
in Victoria and South Australia, and "ONI"
(Ongoing Needs Identification) in New South Wales and
Queensland. In NSW, different applications of the functional
screening items plus assessment level tools (Tier 2),
have also been tested by the The Adult
Training, Learning and Support (ATLAS) reform project.
The various versions of the tools have their associated
reports, guidelines and manuals and these can be accessed
by clicking on the links below.
The most recent version of the ONI
suite of tools was completed for Queensland Health in
June 2004, and at the end of this list of projects and
their documentation there is a generic version of the
manual to accompany the recommended (most up to date)
version of the forms.
