SNAPshot

SNAPshot is software designed primarily to collect “SNAP” (Sub-Acute and Non-Acute Patient) information. SNAPshot has been modified for a range of applications including collecting the ACAT (ACAP), DVA, AROC and HACC Minimum Data Sets.
